MINNETONKA, Minn. -- Managed-care provider UnitedHealth ( UNH) says fourth-quarter profit fell 40% from last year, partly because of a legal settlement. Revenue rose 9% to $20.45 billion from $18.71 billion. The insurer's profit included a charge of 18 cents a share to resolve class-action litigation over out-of-network medical services.
